Who is eligible for this dental plan?
Residents of Alabama who are members of Alabama Farmers Federation.

Why do I need membership in Alabama Farmers Federation to qualify for this dental plan?
This plan is being offered only to members of Alabama Farmers Federation as one of our Member Benefits, therefore you must maintain your membership to enroll and keep this dental plan.

How do I become a member of Alabama Farmers Federation?
If you have Alfa Insurance and live in Alabama, you probably are already a
member.  If you are not a member you may download a Membership Application
and mail it in, or you may go to any local Alfa office and join the Alabama
Farmers Federation and be able to enroll in the Alfa Dental plan.

How do I enroll in the Alfa Dental plan?
You may Enroll Online or you may stop by one of many Alfa Insurance offices across Alabama and they will be glad to assist you.

How does this plan work?
You can choose any licensed dentist; however, you are likely to have lower out-of-pocket costs if you choose from the large network of Delta Dental PPO dentists (Preferred Provider Organization). These dentists have agreed to provide treatment at reduced fees which means your share of the bill will likely be lower.

How do I know if my dentist is Delta Dental dentist?
Delta Dental has two networks, the Delta Dental PPO network and the Delta Dental Premier network.  We recommend that you verify that your dentist is a Delta Dental provider before each visit.  Please be aware that asking if they “accept Delta Dental” does not guarantee that they are Delta Dental dentist.  It simply means they will file your claim for you.  You still may be charged the full fee up front. Be sure to ask your dentist if he or she is a “participating Delta Dental PPO or Delta Dental Premier dentist”

What are the advantages of visiting a Delta Dental Dentist?
Delta Dental dentists will not balance bill you above Delta Dental’s approved
amount. Delta Dental dentists submit all claim forms directly to Delta Dental and
will only require that you pay your portion of the cost at the time of treatment.  Many
Non-Delta Dental dentists require you to pay all of the cost up front and wait for reimbursement.

What are my payment options?
Monthly bank draft or monthly credit card billing.

When will my checking account / credit card be drafted?
When using a credit card or debit card, the first charge will take place the day you enroll in the Alfa Dental plan. Each renewal payment will take place between the 18th and 23rd of the month prior to each due date.

When does my coverage begin?
If your enrollment form is received by the 20th, your coverage begins the first of the following month.

Can I buy a plan just for my child?
No.  Coverage cannot be purchased for a child under the age of 19 unless it is through a family plan.

What is the maximum age for a fulltime student to be covered?
25 years of age.  Coverage will end the last day of the month in which they turn 25.

If I am over age 65, can I enroll?
Yes.

Are orthodontic (braces) services covered?                                                           
No.
